Hey families — are all of your child’s forms for the current school year up-to-date?

Each year, Cicero District 99 needs to ensure that all students’ information is verified in order to ensure accurate records are being kept by our schools! The annual date for student forms to reset is July 1. If you have not completed your annual forms for this year after July 1, you will need to submit them at this time. 

Examples of records which need to be updated annually include:

  • Family and Emergency Contact Information

  • Household Income Form

  • Technology Lending Agreement

  • Parental Consent Form

  • Authorization for Student Photographs and Work Release

  • Requirements for Graduation Participation

  • Student Records Release

  • Technology Acceptable Use Policy

  • Student Safety Contracts

  • and more!

Please take a moment TODAY to log on to your PowerSchool Parent Portal and update all forms for the 2024-2025 school year!

Please note: Some forms are only applicable to specific grade levels, and forms must be completed for each of your children!
