Thank you for taking the time to register with us. Whether you’re a new family joining our community or returning for another great year, we’re here to make the process as smooth as possible.

  • To enroll your child in Cicero District 99, you must confirm that you live within our district boundaries. This helps us ensure that each student attending our schools is eligible to do so. 


     

    PROOF OF RESIDENCY: REQUIRED DOCUMENTS

    To verify residency, your must upload three (3) current documents that show:

    • Your name
    • The address you are registering with (must be within district boundaries) 
    • Date within the last 30 days

     

    These documents can include things like utility bills, lease agreements, mortgage statements, or official mail.

    Important: Before uploading, remove documents from their envelopes and block out any account or Social Security numbers.

    If you’re unable to provide three documents, please contact our registration staff for help. In some cases, a home visit or additional documentation may be required. 


     

    LEGAL GUARDIAN VERIFICATION 

    You must upload a birth certificate. If unavailable, you may submit other proof of identity and birthdate along with a signed affidavit.

     

    What qualifies as a “Legal Guardian”?

    • A natural or adoptive parent listed on the birth certificate
    • A person who was granted court-ordered guardianship
    • A person who receives public aid on behalf of the child
    • A person who has assumed full responsibility for the child and can provide a nighttime abode 

     

    ⚠️  Knowingly providing false residency information is a Class C misdemeanor. Enrolling a non-resident student may result in tuition charges and legal consequences. 

  • To help us complete your child’s registration, please upload the documents listed below. Some are required to begin school, while others may be submitted later or only if requested by school staff.


     

    REQUIRED 

    • A certified, original birth certificate: If the birth certificate is not currently available, you may submit another document showing your child’s identity and date of birth along with a signed affidavit.

     

    You cannot move forward with registration if you do not upload this document. 


     

    HELPFUL TO SUBMIT (IF AVAILABLE)

     

    Don’t worry if you don’t have these right away — our school staff will follow up with you if anything is missing or required.


     

    OTHER DOCUMENTS

    • Parent/Guardian ID may be requested for verification
    • Two (2) proofs of income are only required for certain programs and only if instructed by school staff

     

    These are only needed in specific situations


     

    TIPS FOR UPLOADING

    • Upload clear images or scans of the documents
    • Remove any pages from envelopes before uploading
    • Make sure each file shows the required name, date, and details

  • If you child speaks or hears another language besides English, schools are required by law to assess their English language proficiency. 


     

    HOME LANGUAGE SURVEY

    We ask all families to complete this. These questions help us identify students who may benefit from bilingual or English learner services, such as:

    • Is a language other than English spoken in your home?
    • Does your child speak another language besides English?
    • If yes to either, what language?

     

    LANGUAGE BACKGROUND

    Telling us more about your child’s background helps us understand your child’s language experience and determine if additional language support is needed, such as:

    • Country of birth
    • Country where your child was raised
    • Language your child uses most at home
    • Language(s) parents or guardians most often use with the child
    • Whether you child has a regular childcare provided 

     

    EDUCATIONAL BACKGROUND 

    We also ask about your child’s past educational experiences:

    • Did your child receive support in a Bilingual program at their previous school?
    • Has your child ever received formal education outside the United States?
    • Has your child received Special Education services?
  • As part of of the registration process, Cicero District 99 is required by the Illinois State Board of Education and the U.S. Department of Education to collect information about each student’s race and ethnicity. This helps us report data accurately and ensure equal educational opportunities for all students. Your responses will remain confidential and will not affect your child’s enrollment or services.


     

    ETHNICITY

    You will be asked if your child is Hispanic or Latino. Which includes: Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin — regardless of race.


     

    RACE

    You will be asked to select one or more races that best describe your child from the following list:

    • American Indian or Alaskan Native: Origins in North, South, or Central America with tribal or community ties
    • Asian: Origins in East Asia, Southeast Asia, or the Indian subcontinent
    • Black or African American: Origins in any of the Black racial groups of Africa
    • Native Hawaiian or Pacific Islander: Origins in Hawaii, Guam, Samoa, or other Pacific Islands
    • White: Origins in Europe
    • Middle Eastern or North African: Origins in the Middle East or North Africa
    • Two or More Races: A combination of the above 

     

    Tip: Even if you answered YES to the Hispanic question, you still need to select a race or races. 


     

    ⚠️  This information is kept on file for at least three years as required by law. If there’s a legal or audit-related reason, it may be kept longer. 

  • Cicero District 99 is committed to keeping families informed about their child’s academic progress. In accordance with Illinois State Board of Education (ISBE) requirements, all report cards and progress reports are provided in English and in the student’s home language, unless a parent or guardian requests otherwise.


     

    OPTION 1: BILINGUAL REPORTS

    You can choose to receive your child’s report card and progress reports in English and in your home language. You’ll be asked to select your preferred language(s) from a dropdown menu.


     

    OPTION 2: ENGLISH ONLY

    You can choose to receive report cards and progress reports in English only. You’ll need to sign electronically to confirm this preference.

  • Help us keep your child safe by keeping your contact information up to date. Please make sure all names, phone numbers, and addresses we have on file for are always correct.

     

    Important: To add or remove a parent or legal guardian, please contact your child’s school directly. To authorize someone new to pick up your child, the school office must be notified.


     

    WHAT YOU’LL BE ASKED TO PROVIDE

    For each contact listed, please provide the following:

    • Full Name
    • Relationship To The Student (Mother, Father, Aunt, Neighbor, etc.)
    • Address
    • Phone Number
    • Email Address

     

    You’ll also be asked whether this person:

    • Lives with the student
    • Has legal custody
    • Is allowed to pick up the student from school
    • Should be contacted in an emergency
    • Should have access to student records (data access)

     

    You can list more than one emergency contact to make sure someone is always reachable in case of an emergency. If you’re unsure about what access a contact should have, or if a contact’s role has changed, your school’s main office can help guide you. 

  • Cicero District 99 provides students with access to electronic networks, including the Internet, as a tool to enhance learning, share resources, and support innovation. To ensure a safe and productive experience, students and parents/guardians must review and agree to our Acceptable Use Policy.


     

    PARENT/GUARDIAN AUTHORIZATION FOR NETWORK ACCESS

    You will be asked to give permission for your child to use the District’s electronic networks, including the Internet, for educational purposes. The District uses internet filters to block inappropriate content, but no system is perfect. Parents/Guardians are responsible for setting expectations for their child’s internet use outside of school. Inappropriate use may result in loss of access, disciplinary action, or legal consequences.

     

    You’ll need to confirm that you’ve read and understood the Acceptable Use Policy and agree to hold the District harmless for any inappropriate content your child may access.


     

    STUDENT AGREEMENT

    Students must also read and agree to the Acceptable Use Policy. By agreeing, students will understand

    • All online activity may be monitored by the District
    • Any violation may result in revoked privileges and disciplinary or legal action
    • They are responsible for appropriate behavior online

     

    Students will need to confirm that they have read the Acceptable Use Agreement and acknowledge that they understand the rules and consequences.


     

    INTERNET & DEVICE ACCESS AT HOME

    To help ensure every student has access to digital learning, we ask families to tell us about their technology needs at home. Based on your responses, the District may offer assistance such as free or discounted internet services or loaner devices.

     

    You will be asked if you have reliable Internet access at home and if your child has access to a learning device at home. If you answer “No” to either question, our team will follow up to help provide the support you need. 

  • Cicero District 99 is proud to celebrate and showcase our students’ achievements. From time to time, we may share student photos, videos, or schoolwork in official publications and with local media to highlight awards, projects, performances, sports, and other school activities.


     

    WHAT YOU’RE AGREEING TO IF NO OBJECTION IS FILED:

    Unless you notify us in writing by September 29th, the District may include your child’s photo, image, or schoolwork in: 

    • Yearbooks
    • School newsletters, brochures, and websites
    • Video recordings or media coverage of school events
    • Local newspapers, online news, or other media

     

    Examples include:

    • Sharing a student’s name and photo for an honor roll or award
    • Publishing artwork, poetry, or stories in a school publication
    • Featuring photos from field trips, sports, or concerts 

     

    We will never publish sensitive personal information. Only your child’s name and relevant context (like an award or event) may be included.


     

    HOW TO OPT OUT:

    If you do not want your child’s name, photo, or work shared, you must submit a written request to your child’s school by September 29th. You can opt out at any time after that by contacting your child’s principal.

  • At Cicero District 99, we believe hands-on learning is one of the most powerful ways to teach Science and STEAM (Science, Technology, Engineering, Art, and Math). To keep our classrooms safe, all students must follow specific safety rules and procedures when using lab materials or equipment.

     

    No student may particiapte in lab activities until this safety contract is electronically signed by both the student and a parent/guardian.


     

    WHY SAFETY MATTERS

    In Science/STEAM classes, students may work with chemicals, glassware, preserved specimens, and other lab tools. While these experiences are exciting and educational, they also require care and responsibility.

     

    That’s why we ask every student to follow these general safety rules & important guidelines:

    • Always behave in a safe, respectful, and responsible manner
    • Follow all verbal and written instructions carefully
    • Never eat, drink, or chew gum in the lab
    • Keep hands away from your face, eyes, and mouth
    • Wash hands after using lab materials
    • Only perform approved experiments
    • Wear safety goggles when instructed and keep them on until told to remove them
    • Handle all materials, tools, and chemicals with caution and respect
    • Clean work area and return materials at the end of class
    • Report any accident or injury immediately to teacher
    • Do not enter storage areas or remove any materials from the classroom
    • Dress properly: tie back long hair, avoid baggy clothing, and remove dangling jewelry 
    • Know where the safety equipment and emergency exits are located

     

    Remember: Keeping our labs safe is a shared reposibility!


     

    HEALTH QUESTIONS

    We ask that you let us know if your child wears contact lenses, is colorblind, or has any allergies so we can help ensure their safety. 


     

    PARENT, GUARDIAN & STUDENT AGREEMENT

    We ask parents/guardians to acknowledge that they’ve read the safety contract with their child and agree to support their safe participation in all Science/STEAM activities. 

     

    Students are agreeing to:

    • Follow all safety rules listed above
    • Respect their classmates, teachers, and their classroom environment
    • Take responsibility for their own safety and the safety of others
    • Accept that failure to follow the rules may result in being excluded from class activities
  • As required by the Family Educational Rights and Privacy Act (FERPA), Cicero District 99 must inform you about the types of student information we may release without written consent — known as “directory information.” This is information that is generally not considered harmful or an invasion of privacy is shared.

     

    You do not need to take acton unless you want to opt out of sharing this information.


     

    WHAT IS DIRECTORY INFORMATION?

    • Student’s Name
    • Address
    • Phone Number
    • Email Address
    • Date and place of birth
    • Grade level
    • Dates of attendance
    • Participation in school activities or sports
    • Weight and height (for athletic teams)
    • Photograph
    • Degrees, honors, and awards received
    • Major field of study
    • Most recently attended school or program

     

    WHY THIS INFORMATION MIGHT BE SHARED:

    • Yearbooks
    • Honor roll or award lists
    • Graduation programs
    • Playbills or programs for performances
    • Sports rosters
    • School newsletters or local media stories
    • Class ring or yearbook publishing services

     

    In compliance with federal law, this information may also be provided to military recruiters unless you opt out.


     

    HOW TO OPT OUT

    If you do not want Cicero District 99 to release any directory information about your child, you must submit a written request to the District by September 29th. Once you opt out, your child’s name and information will be excluded from publications like the yearbook, sports programs, or media coverage.

  • Please review the important notices below. These items relate to student safety, attendance, and state reporting requirements. You will be asked to acknowledge that you’ve read and understood them.


     

    VOLUNTARY STUDENT ACCIDENT INSURANCE:

    Cicero District 99 offers optional accident insurance for students through Special Markets Insurance Consultants (SMICI). This can help cover costs in the event of a student injury during school-related activities. For more information or to enroll, visit  the K-12 Special Markets Website


     

    OPTIONAL MILITARY FAMILY STATUS

    In compliance with Illinois School Code, District 99 is required to report the number of students with a parent or guardian who is a member of the U.S. military. You will have the option to voluntarily report if a parent or guardian is actively serving in the armed forces, is currently deployed, or expects to be deployed during the school year.

     

    This information can be provided during registration or at any time during the year.


     

    CONSECUTIVE ABSENCE POLICY

    To ensure accurate enrollment and attendance records:

    • Any student absent for more than 10 consecutive school days may be dropped from enrollment
    • If there are medical or extenuating circumstances, these must be communicated to the school and approved by the Office of the Assistant Superintendent for Educational Services
    • If dropped, the student must re-register and complete the full registration process before returning.

  • Cicero District 99 is committed to keeping all students safe — physically, emotionally, and socially. In accordance with the Illinois State Board of Education (ISBE), we provide age-appropriate lessons on child sexual abuse prevention for students in grades Pre-K through 6. This is required under Erin’s Law

    WHAT YOUR CHILD WILL LEARN:

    District 99 uses the Second Step Child Protection Unit, which provides six structured, age-appropriate lessons of each grade level. These lessons are designed to help children recognize unsafe situations, understand personal boundaries, and learn how to seek help from trusted adults.

    OPTING OUT

    If you do not want your child to participate in any of the Erin’s Law lessons, you must submit a written objection to your child’s school principal by December, 15th. Students with a submitted objection will be excused from these lessons without penalty.

  • Cicero District 99 uses Google Workspace for Education to help students learn, collaborate, and complete schoolwork in a modern digitial environment. These tools include Gmail, Google Docs, Classroom, Calendar, Drive, and more — used by millions of students and teachers across the country.

     

    To use these tools, we must have your permission to create and manage a Google account for your child. Without your consent, we cannot provide access, which may limit your child’s ability to fully participate in digital learning.


     

    WHAT YOUR CHILD WILL USE GOOGLE FOR:

    • Complete assignments and collaborate on projects
    • Sign in to their Chromebook and learning platforms
    • Access school email and digital classroom tools
    • Build essential 21st-century digital skills

     

    WHAT INFORMATION GOOGLE MAY COLLECT:

    • Student name, email address, and password
    • Optional recovery phone number or profile photo (added by the student)
    • Usage activity (files viewed, who they communicate with, etc.)
    • Device and browser info (operating system, IP address, app versions, etc.)
    • Location data based on IP address or GPS
    • Feedback or questions submitted to Google

     

    Google does not use student data from core services for advertising and does not serve personalized ads to K-12 users. 


     

    WHAT SERVICES STUDENTS WILL HAVE ACCESS TO:


     

    YOUR CHOICES AS A PARENT/GUARDIAN

    • Consent to allow your child to use Google Workspace for Education
    • Contact the District to review, manage, or delete your child’s Google account
    • Request restrictions on specific apps or features
    • Manage privacy settings with your child 

     

    If you do not consent, we cannot create a Google account for your child, and access to key educational resources will be limited.

  • Each year, Cicero District 99 asks families to complete this form to help determine eligibility for free or reduced-price school meals and other important benefits. Please submit one form per household, even if your child does not eat school meals.


     

    WHY THIS MATTERS

    • Helps us determine your family’s eligibility for free or reduced meal prices
    • Helps us support your child’s school with federal and state education funding
    • Helps us identify students who qualify for additional support services, such as homeless, migrant, or foster care assistance

     

    Even if you don’t think you qualify, submitting this form helps your school. All information is confidential. 


     

    WHO SHOULD BE LISTED:

    You’ll need to include every child in your household, from infants through 12th grade — even if they don’t attend Cicero District 99. A household member is anyone who lives with you and shares income or expenses, even if they’re not related. For each child you’ll be asked:

    • First Name, Middle Initial, and Last Name
    • School and Grade Level (if applicable)
    • Whether the child is a foster child
    • Whether they have their own income
    • Whether the child is homeless, a migrant, or a runaway
    • Their current meal status (Free, Reduced, or Full Price, if known)

     

    BENEFITS PARTICIPATION

    Please indicate if any household members currently participate in:

    • Food Assistance (SNAP)
    • Temporary Assistance for Needy Families (TANF)
    • Food Distribution Program on Indian Reservations (FDPIR)

     

    You’ll be asked to provide: Street Address (if available), City, State, Zip Code, Phone Number, and Email Address

     

    You will certify that all information provided is true and complete. Understand this form is used to determine eligibility for federal benefits. School officials may verify the information. Providing false information may result in loss of benefits or legal consequences.
