School Supply Lists

Lists are for 2021-2022 school year. Kindergarten through eighth grade.

Dress Code

All students are required to wear heather grey polos and navy blue pants to school every day, unless otherwise noted.

Report Bullying

Health Information

There are certain health requirements that must be met before your child can be admitted to school. Your child will not be admitted to school until the state requirements are met. This policy will be strictly enforced.

Student Physicals

All children entering pre-school, kindergarten, sixth grade and all transfers must provide a physical. Click below to download and print a copy of the form. 

Dental Exams

All children entering kindergarten, second grade, and sixth grade must provide a dental examination by a licensed dentist. Click below to download and print a copy of the form. 

Vision Tests

All children entering kindergarten or entering an Illinois school district for the first time must provide a professional vision examination by an optometrist. 


To be completed and signed by parent/guardian and verified by health care provider. Information on this form may be shared with appropriate personnel for health and educational purposes.  

Action Plans

 Students with chronic medical conditions (i.e. allergies, asthma, diabetes, seizures) are encouraged and/or required to have a current Emergency Action Plan on file at the start of the school year filled out and signed by a licensed health care provider.



The administration of medication during school hours will be limited to those medications prescribed by a licensed care provider and necessary to maintain a student's health and well-being. Should your student require medication to be administered during school hours, a written physician order is required. Medications must be provided in the original, labeled prescription container. 



Students returning to school with health-related restrictions must provide proper documentation from a licensed health professional. Additional information may be required by the school nurse prior to the student's return to school. Please contact your school nurse to discuss additional forms needed.

Cicero District 99 is working in partnership with the Illinois State Board of Education (ISBE) to ensure the District provides the most current resources and guidance to support Cicero education communities regarding the coronavirus disease 2019 (COVID-19) outbreak. As this is a rapidly evolving situation, the District will continue to share up-to-date information as it becomes available.

COVID-19 vaccines are FREE and available to anyone who wants one. District 99 will provide vaccination opportunities at various schools all year long.

FREE COVID-19 tests are provided for all District 99 students during the school year.

No More Letter Grades

At the start of the 2020-2021 school year, District 99 transitioned to a new grading system that breaks down each subject into specific learning targets, and identifies if a student has mastered the standard or still needs work.

Cicero District 99 has remained dedicated to bridging the digital divide through friendly and consistent incorporation of technology. Parents don't have to worry about connectivity; everyone has indicated either having dependable home internet or is provided free internet access via hotspots provided by the District.

The District's Tech Department has established protocols for connection and assistance, promising the District is a 1:1 district with dependable digital practices.

Online Registration

2021-2022 School Year 

District 99 has moved to full online registration. Parents will now be able to register their child at home and at any time during the registration periods. No more taking off work and waiting in long lines — online registration is here! 

Registration is open year round for new students.

Re-enrollment for all current students is currently CLOSED. Please contact your child's school if you have not yet re-enrolled your child for the upcoming school year.


Dual Language Program 

Completing enrollment for the dual program does not guarantee that your child will be admitted to the program. 

Parents/Guardians of Pre-K and Kindergarten students must complete registration for admission to be considered.

Students will be considered in the order in which their entries are received since space is limited.
